IRS Forms 433-A & 433-B, Collection Information Statements

IRS financial statements are the most important forms we use to determine whether or not we can do an Offer in Compromise and how much must be offered. We also use the forms to determine appropriate payment agreement amounts. You can download the forms, which are in Adobe PDF format. If you do not have the Adobe Acrobat Reader installed on your computer, you may download it from the Adobe web site.

IRS Form 433-A is the Collection Information Statement for Wage Earners and Self-Employed Individuals. The 433-A is available as a fill-in. Fill-in forms allow you to enter information while the form is displayed by an Adobe Acrobat 4.0 product and then print the completed form out. Fill-in forms give you a cleaner crisper printout for your records and for faxing. You may, of course, print it out and complete it “manually”.

IRS Form 433-B is the Collection Information Statement for Businesses.

If you (or your spouse) are self-employed or if you own a business, you only need to complete the 433-A. If you are an employer (you have employees that work for you), complete the 433-B. In some cases, you may need to complete both forms. No, it isn’t fun, but in most cases it isn’t that complicated.
